| Where will the study take place? |
Patient identification and
enrollment will take place at each participating medical center. However, once you enroll in the study, the
VA Palo Alto Health Care
System (VAPAHCS) will be the primary location.

| How long is the study? |
The first part of the study is a one day commitment where your lifetime physical activity and health history is assessed. If you choose to enroll in the exercise study participation will be for three years.

| Is my physician involved? |
You will continue to see your regular health care provider as you are normally scheduled.

| Who are the Principal Investigators on the study? |
The Principal Investigator for the study is Ronald L. Dalman, M.D. and the Co-PI is Dr.
Lawrence Leung.
